What Actually Determines Your Coverage
Three things decide what your plan actually pays. Medical necessity. Network status. Level of care. Each one works independently, so all three matter.
Medical necessity comes first. A clinician assesses you or your loved one and documents the diagnosis. That assessment tells the insurer why treatment is needed and at what intensity. Without it, most plans will not approve coverage.
Network status is about the provider, not the person. In-network centers have a negotiated rate with your insurer. Out-of-network care often costs more, and some plans cover it at a lower percentage or not at all. Check this before admission.
Level of care matters too. Detox, residential, partial hospitalization, and outpatient are each priced and approved differently. A plan may cover one level readily and question another.
A few plain terms help here:
- Deductible. The amount you pay out of pocket before your insurer starts contributing.
- Copay. A fixed fee you pay for a covered service, like a set amount per therapy session.
- Prior authorization. The insurer's approval, requested before care begins, confirming the treatment meets their criteria. Skipping it can mean a denied claim.
When these pieces line up, coverage becomes clear. Our staff can review your specific plan and explain what to expect before you commit to anything.
